These forms may be typed online, then PRINTED.
Use the TAB or MOUSE to navigate the form.

Enter Dates as  mm/dd/yyyy
Enter Phone/FAX as  2223334444
When you tab or click out of the box, the numbers will display correctly.

The forms may also be printed and completed by hand.
Note: The instruction boxes on the forms are visible on the screen but will not print.
